The covered entity (CE), Florida Healthy Kids Corporation, reported that its business associate (BA) experienced a cyberattack affecting the protected health information (PHI) of 3,500,000 individuals. The PHI involved included names, dates of birth, email addresses, telephone numbers, addresses, Social Security numbers, and financial information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. Following the breach, the CE terminated its business relationship with the BA. OCR provided technical assistance to the CE regarding the HIPAA Breach Notification Rule.
