The covered entity (CE), Florida Health Sciences Center dba Tampa General Hospital, reported that an employee mailed the protected health information (PHI) of 896 individuals to the wrong recipients. The PHI involved included names, dates of birth, Social Security numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards. Staff were also retrained in its requirements to protect and secure PHI. OCR provided the CE with technical assistance regarding the HIPAA Rules.
