Florida Digestive Health Specialists, the covered entity (CE), reported that it experienced a hacking incident that affected the protected health information (PHI) of 3,205 individuals. The PHI involved included names, date of birth, addresses, drivers’ license and Social Security numbers, medical record numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. The CE implemented additional administrative, technical, physical, and security safeguards.
