The covered entity (CE), Florida Department of Health reported that it experienced a hacking attack that affected the protected health information (PHI) of 729,699 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, conditions, claims information, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
