Florida Agency for Persons with Disabilities

The covered entity, (CE), Florida Agency for Persons with Disabilities, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 1,951 individuals. The ePHI involved included names, dates of birth, addresses, other identifiers, Social Security numbers, and medical information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE sanctioned the responsible employee and implemented additional technical safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
