The covered entity (CE), FC Compassus, LLC, reported that an employee of its business associate (BA) was the subject of an email phishing attack that affected the protected health information (PHI) of 2,703 individuals. The PHI involved includes names, addresses, dates of birth, diagnoses/conditions, and medications. The BA provided breach notification to the CE. The CE notified HHS and the affected individuals.
