Famous Enterprises Employee Benefit Plan, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 528 individuals. The PHI involved included names, addresses, Social Security numbers, claims information, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. All staff were retrained.
