Family Medicine of Weston

Family Medicine of Weston, the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 10,700 individuals. The ePHI involved included names, Social Security numbers, health insurance information, and clinical information. The CE notified HHS, affected individuals, and the media. At the time of the breach, the CE did not have a business associate agreement in place with the BA, and had not completed a risk analysis. OCR provided the CE with technical assistance. In response to the breach, the CE ceased its business relationship with the BA.
