The covered entity (CE), Falcon Healthcare, Inc. dba Interim Healthcare of Lubbock Texas, reported that it experienced a ransomware incident that affected the electronic protected health information (ePHI) of 89,443 individuals. The ePHI involved included names, dates of birth, Social Security numbers, addresses, drivers’ license numbers, other identifiers, claims information, diagnoses and conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI. OCR also provided technical assistance to the CE.
