Fairfax Oral and Maxillofacial Surgery

The covered entity (CE), Fairfax Oral and Maxillofacial Surgery, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 235,931 individuals. The PHI involved included names, addresses, dates of birth, driver’s license numbers, diagnoses/conditions, health insurance information, medical history information and Social Security numbers. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals, implemented additional administrative, technical, and security safeguards to better protect its PHI.
