Eyefinity

Eyefinity, the business associate (BA), experienced a breach when it discovered a vulnerability involving a small portion of its cloud-based data storage environment that compromised the protected health information (PHI) of approximately 1,352 individuals. The PHI involved included names, dates of birth, addresses, and clinical information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA applied administrative and technical safeguards to protect against unauthorized access to its PHI.
