Eye Physicians of Pinellas

Eye Physicians of Pinellas, PA dba The Eye Institute of West Florida, the covered entity (CE), reported that an employee emailed the electronic protected health information (ePHI) of 1,650 individuals to her/her personal email address. The ePHI involved included names, addresses, email addresses, phone numbers and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
