The covered entity (CE), Eye Care Surgery Center, Inc. reported that an unencrypted laptop containing the electronic protected health information (ePHI) of 2,553 individuals was stolen from its facility. The ePHI involved included names, dates of birth, and diagnoses. In response to this breach incident, the CE provided notice to OCR, affected individuals, and the media. Additionally, the CE reported the incident to local law enforcement, encrypted its laptops and workstations, and improved physical safeguards to prevent similar incidents.
