The covered entity (CE), Excellent Home Care Services, LLC, reported that a workforce member sent an email containing the protected health information (PHI) of 16278 individuals to the wrong recipient. The PHI involved demographic and clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach the CE offered free credit monitoring to affected individuals, and implemented administrative, technical and security safeguards and retrained the workforce member on email security. OCR provided technical assistance to the CE.
