EVERSANA

The covered entity (CE), EVERSANA, reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 29,631 individuals. The ePHI involved included names, addresses, birthdates, drivers’ license information, Social Security numbers, treatment information, financial information, claims data, diagnoses/conditions, and medications prescribed. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. Substitute notice was also provided. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff on the proper methods of identifying fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
