Envision Home Health and Hospice

The covered entity (CE), Envision Home Health and Hospice, reported that an employee responded to a phishing email and provided the protected health information (PHI) of 5,381 individuals. The PHI involved included names, email addresses, telephone numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications. OCR provided technical assistance to the CE regarding its HIPAA Security Rule requirements.
