The business associate (BA), EngageMED, reported that it experienced a hacking incident affecting 249,297 individuals’ protected health information (PHI). The affected PHI included demographic, clinical, and financial information. The BA notified HHS, affected individuals and the media. In response to the breach, the BA created a new and/or updated Security Rule Risk Management Plan, implemented new technical safeguards, revised policies and procedures, and trained or retrained workforce members.
