Empath-Stratum Inc. doing business as Empath Health

The covered entity (CE), Empath-Stratum Inc. dba Empath Health reported that it multiple employees were the targets of an email phishing scheme that affected the protected health information (PHI) of 40,569 individuals. The PHI involved includes names, addresses, dates of birth, social security numbers, credit card/bank account information, diagnoses/conditions, medications and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring to affected individuals and implemented administrative, technical and security safeguards.
