Easterseals Delaware and Maryland's Eastern Shore

The covered entity (CE), Easterseals Delaware & Maryland’s Eastern Shore, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 15,208 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
