Easter Seals DC/MD/VA, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 584 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, financial information, and diagnoses. The CE notified HHS, affected individuals, and posted substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. Staff were retrained on email security.
