East River Medical Imaging

The covered entity (CE), East River Medical Imaging, reported that it experienced a hacking attack that compromised the protected health information z(PHI) of 605,809 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were also retrained on obligations to protect and secure PHI.
