East Central Indiana School Trust

East Central Indiana School Trust, the covered entity (CE), reported that an employee was the victim of an email phishing scheme affecting 3,529 individuals. The protected health information (PHI) involved included names, addresses, dates of birth, drivers’ license information, Social Security numbers, diagnoses, medications prescribed, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented administrative and technical safeguards to better protect its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
