The business associate (BA), Dynamic Health Care, reported that it was subjected to a ransomware attack that affected the protected health information (PHI) of 947 individuals. The PHI involved included names and treatment information. The BA notified HHS and the covered entities. In response to the breach the BA implemented additional administrative and technical safeguards. Following the breach, the common owner of the covered entities and the BA informed OCR that it had ceased operations.
