Dr. Robert Carpenter, the covered entity (CE), reported that his practice was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 3,311 individuals. The ePHI involved included names, mailing addresses, dates of birth, Social Security Numbers, clinical diagnoses, and medical conditions. The CE notified HHS, affected individuals, and the media. In response to the breach incident, the CE implemented new administrative and technical safeguards and retrained its staff to better protect the privacy of ePHI.
