Dow Rummel Village

The covered entity (CE), Dow Rummel Village, reported that multiple employees were the subjects of an email phishing attack that affected the protected health information (PHI) of 1,079 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial information, and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary identity protection services and implemented additional administrative, technical, and security safeguards. Staff were trained on email security precautions.
