The covered entity (CE), Docs Medical, reported that it was the subject of a ransomware incident that affected the protected health information (PHI) of 3,146 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, financial and claims information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring to affected individuals and implemented additional administrative, technical, and security safeguards. Staff were retrained.
