Dental Specialists of Minnesota

The covered entity (CE), Dental Specialists of Minnesota, dba The Dental Specialists, reported that several employees were the targets of an email phishing scheme that affected the protected health information (PHI) of approximately 38,442 individuals. The PHI involved included names, addresses. dates of birth, drivers’ license numbers, social security numbers, financial information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided security awareness training to its workforce and enhanced its access controls to better protect its PHI. OCR provided technical assistance to the CE regarding the HIPAA Rules.
