The covered entity (CE), David E. Beneson, DPM, reported that his practice was the victim of a ransomware attack affecting the protected health information (PHI) of 12,542 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. As a result of OCR’s investigation, the CE revised its policies and procedures pertaining to the HIPAA Rules.
