Daniel Bendetowicz, MD, PA, the covered entity (CE), reported that he was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 3,314 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims information, lab results, medications prescribed, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring and identity theft protection services to all affected individuals. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
