The covered entity (CE), D&S Residential Holdings, LLC, reported that several employees were the victims of an email phishing attack that affected the protected health information (PHI) of 2,102 individuals. The PHI involved included names, dates of birth, drivers’ license numbers, Social Security numbers, and financial and clinical information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on email security.
