The covered entity (CE), County of Los Angeles Department of Mental Health, reported that an employee was the target of an email phishing attack that compromised the protected health information (PHI) of 2,334 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional technical safeguards and retrained workforce members to better protect its PHI.
