Counseling and Recovery Services of Oklahoma

The covered entity (CE), Counseling and Recovery Services of Oklahoma, reported that an employee was the subject of an email phishing attack that compromised the protected health information (PHI) of approximately 3,365 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, lab results, and diagnoses. The CE notified HHS, the affected individuals, the media, and provided substitute notice. The CE implemented new administrative and technical safeguards. Staff were retrained in email security.
