Cook Children's Medical Center

The covered entity (CE), Cook Children’s Medical Center, reported that it lost a box of discs containing the electronic protected health information (ePHI) of approximately 2,827 individuals. The ePHI involved included names, dates of birth, and clinical and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to protect its ePHI. All staff members were retrained.
