The covered entity (CE), Conway Regional Health System, reported that employees were the victims of an email phishing attack affecting the protected health information (PHI) of 2,945 individuals. The PHI involved included names, dates of birth, email addresses, medical record numbers, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards and enhanced its security awareness and training programs for its employees.
