The covered entity (CE), Consumer Direct Care Network Arizona, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 504 individuals. The PHI involved included names, claims information, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards. Staff were retrained on email security.
