The covered entity (CE), ConnectiCare, reported that an employee inadvertently mailed the protected health information (PHI) of 1,196 individuals to the wrong recipients. The PHI involved included names, member identification numbers, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative safeguards to better protect its PHI.
