Connally Memorial Medical Center

The covered entity (CE), Connally Memorial Medical Center, reported that it an employee was the target of an email phishing incident that affected the protected health information (PHI) of 1,228 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, addresses, medications, diagnoses/conditions, claims and financial information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring and implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
