The covered entity (CE), Cone Health Medical Group, reported that a mailing error caused the protected health information (PHI) of 1,872 individuals to be mailed to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE switched mailing vendors to better protect its PHI. As a result of OCR’s investigation, the CE implemented additional administrative, technical, and security safeguards. The CE also hired a privacy officer. OCR obtained assurances that the CE implemented the aforementioned corrective actions.
