The covered entity (CE), CompuNet Clinical Laboratories, reported that an autofill function on certain patient registration devices caused the protected health information (PHI) of 6,963 individuals to become viewable by others. The PHI involved included names and dates of birth. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect PHI.
