Colorado Retina Associates

Colorado Retina Associates, the covered entity (CE), reported that an employee was the victim of an email phishing attack that compromised the protected health information (PHI) of 26,609 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical and administrative safeguards. OCR provided the CE with technical assistance regarding its obligations under the HIPAA Security Rule.
