The covered entity (CE), Colorado Allergy & Asthma Centers, PC, reported that its business associate (BA) experienced a breach of its network that affected the protected health information (PHI) of 2,063 individuals. The PHI involved included clinical and demographic information. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals.
