The covered entity (CE), Codman Square Health Center, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 10,161 individuals. The PHI involved included names, addresses, dates of birth, claims information, diagnoses, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE installed multi-factor authentication on all devices, retrained workforce members, and implemented other additional technical safeguards.
