Coastal Plains Community Mental Health Mental Retardation Center

The covered entity (CE), Coastal Plains Community Mental Health and Mental Retardation Center, reported that it experienced a ransomware incident that affected the electronic protected health information (ePHI) of 45,357 individuals. The ePHI involved included names, addresses, dates of birth, social security numbers, financial information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional system monitoring safeguards and revised policies and procedures.
