CLARENDON HEALTH AND WELLNESS CENTER

The covered entity (CE), Clarendon Health and Wellness Center, reported that an employee impermissibly downloaded the protected health information (PHI) of 3,297 individuals from its electronic health record. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, medications, claims information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE transitioned to a new EHR provider, strengthened its security safeguards, and retrained its workforce.
