The covered entity (CE), City of Georgetown Fire Department, reported that an unencrypted tablet containing the electronic protected health information (ePHI) of 1,336 individuals, was missing. The ePHI involved included names, addresses, birthdates, Social Security numbers, drivers’ license numbers, diagnoses, and health insurance information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. During the investigation, OCR provided technical assistance to the CE regarding key aspects of the HIPAA Security Rule.
