City of Conneaut, the covered entity (CE), reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 771 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, health insurance information, diagnoses, and conditions. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect sensitive data.
