Circles of Care

Circles of Care, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 61,170 individuals. The PHI involved included names, dates of birth, addresses, diagnoses, claims information, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained its staff to better protect PHI.
