Cheyenne Regional Medical Center, the covered entity (CE), reported that an employee impermissibly accessed the protected health information (PHI) of 1,652 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards, sanctioned the workforce member involved in the breach, and retrained its employees to better protect its PHI. OCR provided the CE with technical assistance regarding its HIPAA Security Rule obligations.
