Charleston Fire Department

The covered entity (CE), Charleston Fire Department, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 2,583 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional safeguards, including multi-factor authentication, revised its policies procedures, and updated its HIPAA Security Rule Risk Analysis.
