Charleston Area Medica Center, the covered entity (CE), reported that an employee was the subject of an email phishing incident that compromised the protected health information (PHI) of 67,413 individuals. The PHI involved included names, Social Security and drivers’ license numbers, birthdates, addresses, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained its employees on email security precautions.
